A Collection of Lumar Myelographical Findings in the Protrused Lumbar Discs


Abstract
A lumbar myclographical study of 410 cases with diagnosed protrusion of lumbar discs and 42 cases normal cases of 452 Korean male soldiers was made the results were as follows:
1. In the antero-posterior views, the width of the subarachnoid dye column varies were; 14.58mm at the 4th lumbar vertebral level and 12.35mm at the 5th lumbar vertebral level.
2. In the lateral views, the width of the subarachnoid dye column varies were; 10.49mm at the 4th lumbar vertebral level and 12.35mm at the 5th lumbar vertebral level.
3. In the prone position, the epidural soft tissue spaces were; 2. 42mm. at the 4th lumbar vertebral level and 2.64mm at the 5th lumbar vertebral level.
4. The levels of termination of lumbosacral cul-de-sac were; 52.37% at the mid portion of 1st sacrum, 25.96% at the upper margin of 1st sacrum, 18.28% at the 2nd sacrum and 3.39¡Æ% at the interspace of 5th lumbar and 1st sacrum.
5. The shape of termination of lumbosacral cul-de-sac were; 53.72% "U" shape and 46.28% "V" shape.
6. The most common type of protrused lumbar disc was posterolateral protrusion (76.49%).
7. The types of lumbar myelogram were; 2.68% "block", 8.04% "gap", 12.79% "veil-like", 33. 33% "pressure defect", 29.17% "hour glass", and 13.99% "root defect".
8. Using both symptomatic (410 cases) and normal control (42 cases), we found that the sensitivity of myelography was 81.92% (18..08% false-negative) and the specificity as roughly 90.95% (9.05% false-positive).
